• A fault in the cylinder 4 injector circuit or connector
• Low fuel pressure at the cylinder 4 rail
• A clogged or dirty cylinder 4 injector
• A cam or crank signal fault affecting cylinder 4 timing

What to do next

1. Check fuel pressure and the fuel filter
2. Run an injector balance test on cylinder 4
3. Inspect the cylinder 4 injector connector and wiring
4. Check cam and crank position signals
5. Replace the cylinder 4 injector if it fails the balance test
