• Low or dirty transmission fluid
• A slipping reverse clutch or band
• A faulty output speed sensor
• A leaking reverse servo
• A TCM calibration fault

What to do next

1. Check the fluid level and condition
2. Compare input and output speed in reverse on a scan tool
3. Read the commanded and actual ratio for reverse
4. Inspect the reverse servo for a leak
5. Service the fluid and filter if the ratio is off
6. Clear the code and retest
