• A short circuit in the V phase winding of the hybrid generator
• A short to ground in the generator V phase cable
• A failed current sensor on the generator V phase
• Insulation breakdown inside the generator
• A stuck inverter transistor driving the generator V phase

What to do next

1. Inspect the generator V phase cable and connectors
2. Test the generator V phase winding to ground
3. Compare V phase current with the other generator phases
4. Check the generator for overheating or burnt windings
5. Replace the inverter if the V phase transistor is shorted
6. Clear the code and retest
