Check these first

• An open in the O battery voltage sense circuit
• A short to ground on the O sense lead
• A damaged O circuit connector
• A failed cell tap on the O channel
• A sense module driver fault on O

What to do next

1. Read O circuit voltage on a scan tool
2. Test the O sense circuit for an open
3. Check the O connector pins for damage
4. Measure the O tap voltage at the pack
5. Replace the sense module if the O channel is dead
6. Clear the code and retest
